Varicocele Defined

A varicocele is similar to a varicose vein and it causes enlargement of the veins contained within the scrotum. It’s also known as varicocele testicle and usually presents in men who are between 15 and 30 years old.

There are three types of varicocele:

  1. Small: This type is identified only by applying pressure to the abdomen, commonly called the Valsalva maneuver.
  2. Medium: Identified by palpation, no need to bear down.
  3. Large: Identified by visual inspection.

Often, varicocele is asymptomatic and therefore it’s undiagnosed and remains untreated.

When varicocele is present in young men, it can inhibit testicular development and decrease sperm production. In older men, it can cause the testicles to shrink. This condition usually exists in the left testicle but decreases sperm production in both testicles.

Symptoms of Varicocele

Varicocele may be present without symptoms although sharp pain may occur in rare instances. Symptoms include:

  • Dull to sharp pain or discomfort
  • Increased discomfort after physical exertion or long periods of standing
  • Increased discomfort at the end of the day
  • Decreased discomfort when lying in a supine position
  • Swelling or bulge
  • Feeling of heaviness in the scrotum

The exact cause of this condition is unknown at present, but it’s thought to be due to improper valve function inside the veins and may start in puberty. Taller men are more prone to develop varicocele but overweight or obese men have a lower incidence of varicocele.

Asymmetrical anatomic structure can cause varicocele as can compression of the renal vein by the kidney. Since the presence of varicocele increases the scrotal temperature, this may account for the decrease in sperm viability.

  • Diagnosed in 15 percent of the adult male population
  • Diagnosed in 40 percent of adult men with fertility issues
  • Likelihood of development increases until adulthood
  • Rates are higher at adolescence
  • Occurs in less than 1 percent of males from 2 to 6 years old
  • Presents in 1 percent of males from 7 to 10 years old
  • Presents in less than 8 percent of males from 11 to 14 years old
  • Presents in 15 percent of males 15 years and older

Post-surgical recurrence of varicocele occurs in about 45 percent of cases. Other complications include infection, scarring, pain, testicular artery damage, arrhythmia, and more.
